Point Person
Acting as point person between all care providers — physical therapists, dentist, audiologist, etc. Making sure everyone is on the same page, and understands your needs.

Educating about Alzheimer’s disease and other forms of dementia — “My person was recently diagnosed with Alzheimer’s disease, now what?”
Education
Providing additional training, coaching and support to family members and home caregivers.
