Sunwise Group is a care management company located in New York's Mid-Hudson Valley. Our licensed clinicians partner with older adults and people living with life threatening illness. We navigate complex healthcare systems, promote healthy aging, and help increase our clients' quality of life. We offer in-person services throughout the Mid-Hudson Valley, and virtual services to all. Learn more about the types of services that we provide here.     

Sunwise is led by Registered Nurse and Certified Case Manager Ellis Traver and Licensed Social Worker Mandy Osborne. Ellis and Mandy are trained professionals who prioritize client autonomy, independence, and well-being.

Sunwise is both a professional and highly personal endeavor. It is inspired by experiences supporting older adults in our own families. As clinicians we are committed to embodying Nursing and Social Work values, principles, and code of ethics as we serve each client.

  • Mandy Osborne, LMSW, MBA

    Mandy is a Licensed Social Worker and Certified Dementia Practitioner who pursued a degree in Gerontology from Columbia University School of Social Work to serve older adults.

    Formerly Mandy staffed the national Alzheimer’s Foundation of America helpline, connecting with people impacted by Alzheimer’s disease or other forms of dementia across the U.S. and internationally. On the helpline she offered resources and referrals, education about dementia, shared best practices, and engaged in non-clinical counseling.

    Mandy has experience serving diverse elder populations including women living with disabilities and chronic illness, Holocaust survivors, BIPOC elders, and members of the LGBTQIA+ community. She is a seasoned care manager, an enthusiastic training facilitator, and a thoughtful support group leader. Former clients described her as kind, curious, collaborative, and resourceful.

    Mandy is also a trained death education counselor, and has a Certificate in the Art of Dying: Integrative Thanatology from the Open Center. She received her Master of Business Administration degree from Simmons School of Management.

    Mandy enjoys pilates, spending time with her cat, discovering new restaurants and food trucks, and being in nature. She is slowly learning how to knit and garden.

  • Ellis Traver, BSN, RN, CCM

    Ellis is a Registered Nurse whose nursing career has been dedicated to serving older adults and people living with life threatening illness. He is a Certified Case Manager through the Commission for Case Manager Certification, an Advanced Professional Member of the Aging Life Care Association, and a Certified Dementia Care Partner.

    As a clinician, he strongly adheres to nursing best practices. Ellis has been described as calm, warm, caring, and a thoughtful problem solver. He has experience with all aspects of care management including conducting nursing assessments, creating care plans, educating clients and families, developing interventions, acting as a liaison with interdisciplinary medical teams, supervising medications, coaching caregivers, optimizing insurance benefits, and much more. He serves diverse clients in private homes, hospitals, rehabilitation settings, and long-term care settings.

    Ellis is passionate about music, and the positive impact of music and sound on health and healing. He has a Sound Healing Certificate from the Integrative Sound & Music Institute from the Open Center.

    Ellis is also a trained death education counselor, and has a Certificate in the Art of Dying: Integrative Thanatology from the Open Center. Ellis received his Bachelor of Science in Nursing from the University of Texas at Arlington, and RN degree from Phillips School of Nursing at Mount Sinai Beth Israel.

    Ellis enjoys hiking, reading fiction and non-fiction, playing music, and researching classic cars. He is currently learning more about architecture and design.

What does Sunwise mean?

The Sunwise Turn refers to an ancient Gaelic belief that the path of the sun moving from east to west — clockwise — was the lucky and prosperous course. In Ireland and Scotland tracing a clockwise path was integrated into both daily life and important rituals. All types of ceremonies included walking clockwise three times to help generate good luck: healing ceremonies, blessing children, weddings, funerals, and more. This practice was present in the tasks of daily life as well. Sailors cast their boats off in a sunwise direction to help ensure safety and a good catch. Tying knots, ploughing, casting fishing nets, driving in screws, milking cows all were completed in a clockwise direction whenever possible.

For us, Sunwise is a reminder of a meaningful tradition practiced by our Gaelic ancestors, which was fueled by a profound connection to nature. As care managers, we are inspired by the Sunwise Turn. Together with our clients, we help identify the most positive way forward, the best path. Then we move on this path together, in a Sunwise direction, to help increase well-being and peace of mind.

We recognize that many older adults in our communities are underserved and experience systematic discrimination based on their socioeconomic status, race, sexual orientation, and/or gender expression.

A percentage of Sunwise Group's profits will be donated to organizations whose mission is to serve LGBTQIA+ and BIPOC elders. This includes groups such as SAGE, the National Caucus and Center on Black Aging, and the National Asian Pacific Center on Aging among others.
