Aging Services + Care Management | Hudson Valley + Virtual
We start with a conversation, a free consultation. We get to know you, and why you reached out to us. This conversation, combined with our clinical training and experience, leads to an initial assessment and recommendations about ways we can help.
The ways we work together, the frequency of how often we help, is up to you — one time, weekly, monthly, or as needed (see below). You decide what works best for you.
Once we are working together, we implement our plan. We stay in touch, with regular updates and observations. We reassess and tweak our plan with your input when needed.
